Cardiologists of Scottsdale accept many insurance plans, Medicare, Visa, MasterCard, and American Express as well as cash and check. Please look at the About page to view our in office and in hospital procedures.
Retirement:
Dr. Cook and Cardiologists of Scottsdale will be retiring on January 1, 2022. The last appointments were scheduled Nov 18, 2021. We are currently discussing an arrangement that may keep the current location open, preserving the small office experience and convenience, with a group of trusted physicians. CVC a well respected cardiology group will probably reopen his office location in 2022. Stay tuned.
We will not be able to continue to refill prescriptions after the office closes. You will need to find a different physician to continue your medications. For many, the Primary Care Physician may be able to continue your chronic established therapy. You will need to ask. Please don’t wait till your supply of medication is exhausted.
We are still working on our long term record maintenance so we can seamlessly send (fax) your records to the Doctor(s) of your choice. Transfer of records will be free of charge. There will be a fee for records not sent directly to your health care provider.

We are not accepting new patients because of our upcoming retirement. Dr. Cook has decreased his rounding at the HonorHealth Scottsdale HealthCare (SHC) hospitals. This was originally due to Covid-19 but has continued as circumstances have changed.
Dr. Cook's primary hospital has been HonorHealth Thompson Peak Campus. Recently, his in-hospital patients are being covered by colleagues in other practices.
